Every employer shall keep an accident book readily accessible in the given format in which the appropriate particulars of any accident causing personal injury to an insured person may be entered.Preserve every such book, when it is completed, for a period of five years from the date of the last entry thereon

Who and When?

Users can get the Form-11 to furnish accident injury details. This form is provided by the Employees’ State Insurance Corporation, Ministry of Labour and Employment, Government of India.
